NEW PATIENTS

Thank you for your interest in becoming a patient of Sports Activity Medicine. Please note that because we are a family practice whose primary emphasis is in sports medicine, we are only able to see new patients with sports medicine related needs at this time. We are not currently seeing new patients for auto accidents, L & I claims or Medicare.
